Find the Patron Collection Complaint Form in the Miscellaneous Binder on the Binder shelf.

The patron should complete the form with their contact information and information about the Library's material that wish to complain about.

In the folder there is a copy of the Policy of Complaints Concerning Library Material. Please make a copy of the policy if the patron wishes to have it.
